Summary:
The Clinician provides counseling therapy, crisis intervention, and/or case management services in an in-home and/or community setting. The Clinician cares for the needs of the specific patient population and provides appropriate direct patient care through assessment, diagnosis, and treatment of specific conditions.
Job Responsibilities:
- Provides counseling, therapy, and/or psychotherapy to clients and families as appropriate to the position; prepares treatment plans, discharge plans, and follow-up care programs; provides therapeutic crisis intervention and urgent services as required.
- Collects data about patients through interview, case history, psychological tests, and/or observational techniques; evaluates data to identify causes of problems and to determine proper therapeutic approach or referral to other specialists.
- Consults with mental health and medical staffs, as appropriate, in the performance of therapeutic and/or casework; refers clients to appropriate service agencies as required.
- Refers clients to appropriate primary care and social service agencies for medical care, financial assistance and other required services
- Follows up to determine reliability of treatment used; changes method and degree of therapy when indicated.
- Establishes and maintains case files, referrals, and other related documents for the treatment of clients; prepares related documentation for civil commitment hearings or other legal proceedings; keeps abreast of patient data to ensure appropriate treatment and care are delivered.
- Consults with other legal and treatment agencies and individuals in relation to patient/client records, rights, and responsibilities.
- Provides supervision to therapeutic mentors and paraprofessionals.
- May provide consultation and in-service training to relevant professionals in immediate and proper handling of and/or referral of a variety of matters.
- Actively participates in staff meetings, in-service training, etc.
- Follows established agency policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
- Ability to work both independently and in a team.

Job Requirements:
Master’s Degree in Social Work, Psychology, Counseling Psychology, Psychiatry, Mental Health Counseling and related fields
- Proficiency with use of computer (Microsoft Office) as communication and data management tool.
- Ability to observe, asses, and record symptoms, reactions, and progress.
- Knowledge and understanding of mental health counseling principles, methods, procedures, and standards.
- Familiarity with psychological testing.
- Interviewing and psychological/developmental evaluation skills.
- Skill in preparing and maintaining patient records.
- Knowledge of clinical operations and procedures.
- Knowledge of community mental health resources and willingness to reach out to existing resources.

Registered Nurse, Adult Foster Care

Summary:
The RN is accountable and responsible for the practice of professional nursing, delivering quality care through the application of the nursing process. The RN provides the essential level of professional nursing needed by each member, and works with management staff, and other personnel and volunteers to ensure quality of care at all the times. The RN must be on site full time, to correspond with staffing ratios.
Job Responsibilities:
- Completing and signing the designated screening tool as required;
- Completing the MDS-HC or CDS and other nursing assessments and coordinating all other applicable clinical assessments;
- Developing and reviewing on an ongoing basis each member’s AFC plan of care;
- Selecting, training, evaluating, and supervising AFC caregivers in conjunction with the care manager;
- reviewing the physician summary;
- Monitoring the health status of all members to ensure that all needed AFC is properly delivered;
- Reporting changes in the health status of any member to the member’s physician;
- Periodically reviewing AFC caregiver logs;
- Conducting on-site visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the care manager) for Level I, and monthly or more often as the member’s condition warrants for Level II;
- Completing a nursing progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants;
- Submitting a semiannual health-status report to the member’s primary care physician;
- Planning for and implementing discharges from the AFC program;
- Conducting an orientation for each AFC caregiver before the AFC caregiver begins personal care; and
- Providing ongoing training to AFC caregivers on health and aging.
Job Requirements:
- Provide and/or supervise basic skilled nursing for each member;
- Supervise other healthcare staff;
- Assist as necessary in the delivery of other program services;
- Train AFC staff;
- Review and implement physician orders;
- Assure that notes are written monthly, and address significant changes in member conditions.
Therapeutic Mentor

Summary:
The Therapeutic Mentoring strives to connect at-risk youth with adult therapeutic mentors (Mental Health Rehabilitation Specialist) who serves as a positive role model, provides support and assistance, and engages the child in enriching community-based experiences. The goal of our program is to assist disadvantaged youth develop coping skills in order to navigate some of the most challenging life circumstances. Our Therapeutic Mentor works one-on-one with the youth, its family and individual therapist to implement treatment goals under the supervision of a behavioral Health Specialist.
Job Responsibilities:
- Assists the youth in communicating his/her needs to the treatment team when ageappropriate.
- Assists with developing, monitoring, and implementing an appropriate treatment plan with measurable outcomes;
- Develops and maintains policies and procedures relating to all components of Therapeutic Mentoring Services;
- Offers services in the youth’s home and community;
- Delivers services in accordance with an existing outpatient or In-home therapy treatment plan that is jointly developed by the outpatient, or In-home therapy provider with input from the parent/guardian/caregiver, and youth whenever possible;
- Supports, coaches and trains the youth in order to support linkages to community resources and services that will sustain the youth’s optimal functioning in the community;
- Supports, coaches and trains the youth in connecting with existing treatment providers and in linking with new or additional resources.
- Supports, coaches and trains the youth in connecting with existing treatment providers and in linking with new or additional resources.
- Participates with State agencies with regards to service/care planning and coordination and planning for transition to adulthood;
- Contributes the voice of the youth in the absence of the youth;
- Attends personnel meetings including (wrap-around meetings, staff meetings, and inservice trainings and supervisions);
- Collaborates closely and effectively with all other members of the MDT;
- Elaborates the documentation of all contacts made with clients and collaterals;
- Monitors and documents client progress;
- Participates in quality assurance and improvement projects related to the program;
- Participates in program planning and expansion services;
- Other relevant duties will be completed as assigned.
Job Requirements:
- Must be 21 years of age or older;
- A bachelor’s degree in a humans services(Social Work, Psychology, Sociology, Criminal Justice, Education, Human Development) field from an accredited university and one (1) year experience working with children/adolescents/transition age youth;
- An associate’s degree in a human services (Social Work, Psychology, Sociology, Criminal Justice, Education, Human Development) field from an accredited school and one (1) year of experience working with the target population; or
- with children/adolescents/transition age youth;
- Ability to work as an integral member of a multidisciplinary team;
- Ability to work with racially diverse clients is a must;
- Excellent communication and writing skills. Knowledge of child development models, DSM IV diagnoses, crisis intervention and behavior management techniques.
